Eosinophils are effector cells that play a role in allergic and nonallergic inflammation and mediate host immune response to parasites, bacteria and viruses. Eosinophil cationic protein (ECP) is a protein with ribonuclease activity that is released during eosinophil activation. It has been attributed with cytotoxic, neurotoxic, fibrosis-promoting and immune-regulatory functions. ECP plays a role in regulating mucosal and immune cells and may directly fight parasitic, bacterial and viral infections.
ECP is a diagnostic marker of asthma. The level of ECP correlates with the level of inflammation in asthmatic patients. Changes in serum ECP level have also been observed in other atopic diseases like allergic rhinitis, and in conditions not related to the eosinophil inflammation, such as bacterial sinusitis.

ECP test measures the level of Eosinophil Cationic Protein (ECP) in serum.
This test requires a blood sample. A tourniquet (elastic) band is placed tightly on the upper arm. The patient is then asked to make a fist. This helps in the build-up of blood filling the veins. The skin is disinfected before needle insertion and the blood sample is collected in vacutainer.

A positive ECP test indicates that there is eosinophil activation in the body which suggests an inflammation either due to asthma, atopic dermatitis, infections, or autoimmune conditions. Further confirmatory testing is indicated if ECP is positive. A negative result indicates that differential diagnoses must be considered.
