Androgen Profile Test Overview

The androgen profile test evaluates key hormones that regulate male and female reproductive health. It measures total and free testosterone, androstenedione, dihydrotestosterone (DHT), sex hormone-binding globulin (SHBG), and the free androgen index (FAI). These hormones influence functions such as libido, muscle mass, hair growth, and menstrual cycles. Abnormal levels may indicate conditions like polycystic ovary syndrome (PCOS), adrenal disorders, male pattern baldness, or infertility.

The androgen test is typically recommended for symptoms like excessive hair growth, acne, or reproductive health concerns. Preparation may require fasting, and results should be reviewed with a doctor to determine the appropriate diagnosis and treatment.

Androgen Profile is a series of hormonal tests performed to detect or rule out gonal dysfunctions, adrenal insufficiencies and certain tumours.

Reasons for Undergoing the Androgen Profile Test

The androgen profile test is a comprehensive blood test that evaluates levels of various male hormones in the body. There are several reasons why you might need to undergo the androgen test:

  • To diagnose hormonal imbalances such as hypogonadism in men (low testosterone) or hyperandrogenism in women (excess androgens)

  • To investigate reproductive issues like infertility, erectile dysfunction, or low libido in men, and infertility or Polycystic Ovary Syndrome (PCOS) symptoms in women

  • To evaluate delayed or early onset puberty in children and investigate cases of ambiguous genitalia

  • To assess adrenal gland function and detect any abnormalities or tumours that might produce excess hormones

  • To monitor the effectiveness of testosterone replacement therapy or antiandrogen therapy

List of Parameters Considered During the Androgen Profile Test

The androgen profile test measures hormones like total and free testosterone, SHBG, DHT, FAI, and androstenedione. These provide insights into androgen levels, helping diagnose conditions like hormonal imbalances, male pattern baldness, prostate issues, and other androgen-related health concerns.

Androgen Profile Test Preparation

To prepare for the androgen profile test, it is typically recommended to fast overnight before the blood draw. Discuss any medications, supplements, or hormones you are taking with your healthcare provider, as these can impact test results.

Androgen Profile Test Results & Interpretation

The results of an androgen profile are analysed by evaluating the levels of each hormone measured in the test.

  • Hormones such as testosterone, DHEA, and androstenedione are assessed to identify any abnormalities.
  • Elevated or reduced levels may indicate a hormonal imbalance or signal specific medical conditions such as polycystic ovary syndrome (PCOS), adrenal disorders, or issues related to reproductive health.
  • These findings are considered alongside the patient’s symptoms and medical history for a comprehensive evaluation.
  • Accurate interpretation of the results is crucial for diagnosing underlying conditions and determining the appropriate treatment or management plan. Always consult your healthcare provider for guidance.

Frequently Asked Questions

Androgen Profile test measures the testosterone, SHBG, DHT, Free Androgen Index, androstenedione levels in the blood.

The test is done:

  • To evaluate androgen excess or deficiency related to gonadal function, adrenal function, or tumor activity.
  • Helpful in children to investigate delayed or precocious puberty and with ambiguous genitalia.
  • Helps in monitoring testosterone replacement therapy and antiandrogen therapy.

People with the following symptoms:

  • Delayed development of secondary sexual characteristics
  • Gonadal dysfunctions in males and females.
  • Females with PCOs like symptoms, infertility
  • Erectile dysfunctions, testicular tumours.

This test requires a blood sample. A tourniquet (elastic) band is placed tightly on the upper arm. The patient is then asked to make a fist. This helps in the build-up of blood filling the veins. The skin is disinfected before needle insertion and the blood sample is collected in vacutainer.

Androstenedione

It is increased in polycystic ovarian disease, hirsutism, virilization, congenital adrenal hyperplasia, adrenal and ovarian tumours and Cushings disease, pregnancy and exercise.

It is decreased in Addison's disease.

Testosterone Test

In males high levels are seen in testicular tumors, and adrenal tumor.

Low levels are seen in Testicular failure or inadequate stimulation by pituitary gonadotropins, Infertility, Erectile Dysfunction, Delayed puberty, Early puberty and cancer treatment.

In females high testosterone levels are seen in Polycystic ovarian syndrome (PCOS), Adrenal tumor, Congenital Adrenal hyperplasia (CAH) and idiopathic hirsutism.

Low levels are seen in Primary & secondary hypogonadism, Testicular feminization.

SHBG Test

High values of SHBG are found in hyperthyroidism, hypogonadism, androgen insensitivity and hepatic cirrhosis in men.

Low concentrations are found in myxoedema, hyperprolactinaemia and syndromes of excessive androgen activity.

DHT- Dihydrotestosterone Serum (DHT)

High levels of DHT may occur due to idiopathic hirsutism, PCOS and prostate cancers.

Low levels may occur due in Klinefelter's syndrome, patient with germinal cell aplasia and azoospermia, and anorchia.

Free Androgen Index (FAI)

A high free androgen index value indicates that there is too much biologically active testosterone in the blood. This may indicate hormonal imbalance.

Low-free androgen index indicates testosterone deficiency. Results should be further evaluated in consultation with the treating clinician.

Bioavailable Testosterone Test

If the results are elevated or low it suggests hormonal imbalance. Results should be correlated with the clinical findings as well.

  • Testosterone test
  • SHBG test
  • DHT test
  • Androgen test
  • Bioavailable testosterone test
  • Free Androgen Index Test [FAI]

This is a major adrenal androgen also produced by the testes and ovaries.

Testosterone is the principal androgen in men and found in small amounts in women as well. Testosterone aids in the development of secondary sexual characteristics like enlargement of the genitals, body hair growth, development of muscle and deepening of the voice

SHBG is an important transport protein for estrogens and androgens in peripheral blood. SHBG concentration in plasma is regulated by androgen/estrogen balance, thyroid hormones, insulin and dietary factors.

FAI is ratio of testosterone to SHBG which helps in identifying excessive androgen activity & useful in the evaluation of mild disorders of androgen metabolism.

DHT is a steroid similar to testosterone and androstenedione, which belong to a class called androgens. It is a potent Androgen derived from testosterone. In males it is responsible for the male sexual characteristics, in females it is converted to estradiol which is responsible for the development of the reproductive organs.

Testosterone bioavailable test is useful to assess the overall effect of testosterone hormones especially in patients with known abnormalities or on hormone therapy.

An androgen profile test checks key male hormones like testosterone and DHT, which impact everything from reproductive health to muscle growth and hair. It helps diagnose hormone imbalances, monitor therapy, and address issues like PCOS, infertility, and male pattern baldness. If you're experiencing symptoms like acne, irregular periods, or excess hair growth, this test could provide answers. It’s also great for tracking puberty changes and treatment results.

Yes, home sample collection is available for the androgen test. Many labs, like Metropolis offer online booking services and home sample collection services for your convenience.

Normal levels for key parameters include total testosterone (300-1,000 ng/dL for males, 15-70 ng/dL for females), free testosterone (a fraction of total testosterone), SHBG (varies widely), DHT (interpreted in context), FAI (calculated based on total testosterone and SHBG), and Androstenedione (interpreted in context).

The androgen profile test is used to diagnose and monitor conditions such as hypogonadism, PCOS, and adrenal gland disorders, and to evaluate physical and sexual development in children. It also helps in monitoring testosterone replacement therapy and antiandrogen therapy.

The frequency of testing depends on the specific health condition being monitored or diagnosed. It is typically done as needed based on clinical symptoms or as part of regular monitoring for conditions like hypogonadism or PCOS.

The androgen profile test can be done at any time, but it is usually recommended in the morning when hormone levels are typically more stable.

Fasting is usually not required for an androgen profile test, but it’s best to follow any specific instructions from your healthcare provider.

If the results are abnormal, additional tests may be recommended to further investigate the cause of the hormonal imbalance. Once a diagnosis is made, an appropriate treatment plan will be implemented.

The androgen profile test parameters include total testosterone, free testosterone, SHBG, DHT, FAI, and androstenedione levels.

The androgen profile test involves a standard blood draw procedure where a healthcare provider cleans an area of your arm, applies a tourniquet, and inserts a needle into a vein to collect a blood sample. The sample is then sent to a laboratory for analysis.

Yes, certain medications, including hormone therapies, can impact your androgen profile test results. It is important to discuss any medications with your healthcare provider before the test.

The androgen profile test helps guide your treatment plan by providing important information about your hormone levels. This can be useful in diagnosing various medical conditions, monitoring the effectiveness of treatments, and providing insights into symptoms or issues you may be experiencing.

The androgen profile test should be done when symptoms of hormonal imbalances or related health issues are present or as part of regular monitoring for conditions such as hypogonadism or PCOS.

The androgen profile test process typically takes only a few minutes.

The turnaround time for an androgen profile test report may vary depending on the lab and location. Typically, results are available within 24 to 48 hours based on the day/time the sample was collected and processed. However, it's best to check with your healthcare provider or the diagnostic center for specific timelines.
