This test requires a blood sample. A tourniquet (elastic) band is placed tightly on the upper arm. The patient is then asked to make a fist. This helps in the build-up of blood filling the veins. The skin is disinfected before needle insertion and the blood sample is collected in vacutainer.

In anemia, the peripheral blood smear examination may reveal the following:

  • Anemia caused by deficiency of iron, vitamin B12, folic acid will show large, immature red blood cells, known as macrocytes, and a reduced number of red blood cells (anemia).
  • Anemia caused by hemolysis (destruction of red blood cells) will show a high number of red cells fragments, known as spherocytes, and red cells of different size, shape and color.
  • Anemia caused by aplastic anemia or bone marrow failure will show a reduced number of red blood cells and an increase in the number of immature white blood cells and platelets, known as nucleated red blood cells.
