Celiac disease (CD) is an immune-mediated enteropathy induced by dietary gluten. 
When a genetically predisposed individual consumes food containing gluten like  wheat, barley, rye, and other grains ,inflammation and damage of the small bowel mucosa occurs which leads to malabsorption , loss of bone density, miscarriage, infertility or even neurological diseases or certain cancers. Gliadin IGG test is a non invasive , quantitative determination of IgA antibodies against deamidated gliadin protein epitopes (DGP) in human serum or plasma.

Overview

 

Gliadins are proteins present in gluten-containing cereals like wheat, barley, rye, and other grains. IgA and IgG antibodies against gliadins are found in the sera of patients with celiac disease (CD). On reaching the small intestine, transglutaminase deaminates gliadins. This increases the binding of gliadins to MHC class II molecules. Gliadins get attached to these antibodies and damage the small bowel mucosa by triggering inflammation.

 

What is the DGP Serum Test?

The DGP serum test is a non-invasive, quantitative test that detects de-amidated IgG gliadin antibodies in the sera of untreated CD patients. These antibodies are nonspecific for celiac disease. They are also found in other gastrointestinal diseases. However, IgG gliadin antibodies are preferred in children less than 2 years, in people with IgA deficiencies, and in cases of equivocal tTG IgA results.

  3. Evaluate patients with osteopenia, osteoporosis, or osteomalacia. Although these clinical features are seen in metabolic bone disease, they may occur due to calcium malabsorption.
  4. Evaluate iron and folate deficiencies in patients with no classical CD features.
  5. Make the diagnosis of CD in children less than 2 years and people having IgA deficiency.
  6. Make the diagnosis of CD in cases showing equivocal tTG IgA results.
  7. Monitor the patient’s adherence to a gluten-free diet.

Quantitative assessment of  Gliadin-IgG Antibodies de-amidated (dGP) in serum. The IgG antibodies make it possible to determine the level of disease activity and to monitor adherence to a gluten-free diet.

It detects de-amidated gliadin IgG antibodies in the sera of patients untreated for celiac disease. This test is non-invasive. It quantitatively determines IgA antibodies against deamidated gliadin protein epitopes DGP in human serum or plasma.

Gliadin igg test requires a blood sample. A tourniquet (elastic) band is placed tightly on the upper arm. The patient is then asked to make a fist. This helps in the build-up of blood filling the veins. The skin is disinfected before needle insertion and the blood sample is collected in vacutainer.

Gliadin-IgG Antibodies de-amidated (dGP) in serum is increased (>25RU/mL) in people with Celiac disease who have an extreme allergy to food containing gluten. It also helps in monitoring the effect of therapy and gluten free diet and can be done as a substitute to invasive biopsy procedures.

A normal DGP Serum Test does not rule out celiac disease. Hence, the doctor will advise a permissive haplotyping or duodenal biopsy with histological evaluation to confirm celiac disease.

If the DGP test is abnormal, the doctor will advise you to exclude gluten from your diet.

The DGP Serum test is done in patients showing symptoms of malabsorption and celiac disease. It is more commonly done in children less than 2 years, people with IgA deficiencies, and in cases of equivocal tTG IgA results.

A DGP test is done in patients having the symptoms of malabsorption.

This test requires a blood sample. An elastic tourniquet band will be wrapped tightly on your upper arm. The laboratory technician will ask to make a fist. This helps in the build-up of blood filling the veins. Before needle insertion, your skin will be disinfected. Finally, your blood sample will be collected in a vacutainer.

 

Do not consume a gluten-free diet before undergoing this test, as it will give a false negative result. Consume a normal diet.

For patients who are already consuming a gluten-free diet, a DGP Serum test won’t be able to detect de-amidated gliadin IgG antibodies in the serum. It can only diagnose CD in untreated patients.

The DGP Serum Test results are reported as a titer. A high titer is more specific for CD compared to a low titer. If the de-amidated gliadin IgG antibodies in your serum are increased to >25 RU/mL, then the diagnosis of CD will be made.
